Elena's Home 1 

Elena's Home 1 is a 6 bed residential facility. In addition to accepting Private Pay or MedicAid residents, this home can also accept ECS or SBS residents. This is also the home base of one of the main AFH Provider.

Elena's Home 3

Elena's Home 3 is a 6 bed residential facility. We accept Private Pay or MedicAid residents, and/or people with Dementia or Mental Health. This home also has a training center which educates our internal staff and any future quality caregivers.

Kirsten Care Adult Family Home

Kirsten Care AFH is a 5 bed residential facility. In addition to Private Pay, MedicAid, Dementia, and Mental Health clients, we can also accept residents who have Developmental Disability. This is also the home base of our Registered Nurses.

Nashville Adult Family Home

Nashville AFH is a 6 bed residential facility. We accept Private Pay or MedicAid residents, and/or people with Dementia or Mental Health. This is also the home base of our resident RN Delegator.
